PATHOLOGIC QUIZ CASE 1

A 26-year-old Puerto Rican man presented a one-month history of indolent progressive bilateral parotid gland enlargement without previous history of similar episodes or signs of infection. Associated with his main complaint were a 9.04-kg (20-lb) weight loss, a sensation of weakness, occasional shortness of breath, and conjunctival itching. On palpation the parotid glands were markedly enlarged, firm, and nontender with normal clear salivary flow. Results of the rest of the examination were unremarkable. Results of a complete blood cell count, urinanalysis, and protein electrophoresis test were normal. The calcium level was 10.7 mg/100 ml and total serum protein level was 8.4 gm/100 ml. Results of a purified protein derivative test were normal. A chest x-ray film (Fig 1) and a biopsy specimen of the parotid tissue (Fig 2) are shown.
