Experiences With the Long Anterior Cervical Flap Incision

A VARIETY of neck incisions have been proposed for block dissection of the neck. This reflects the unsuitability of any single neck incision for all situations. The standard double or single "Y" incision, which gives excellent exposure, has the drawback,

CONLEY-1966 particularly in individuals who have had irradiation, of necrosis and skin loss in the critical area of the carotid bifurcation. Most often the area of necrosis is associated with the posterior skin flap in this incision.

Many interesting variations in incisions to avoid necrosis in this area have been proposed. The incision utilized in this experience affords good protection from necrosis in the area of the carotid bifurcation and there has been no instance of carotid exposure in 50 consecutive cases.
