PATHOLOGIC QUIZ CASE 1

A 17-year-old boy gave a history of progressive nasal obstruction and intermittent bleeding from the right side of his nose for three months. There was no previous history of nasal difficulty nor any symptoms suggesting allergy in his family.

Examination revealed that his mouth breathing and rhinolalia were due to complete bilateral nasal airway obstruction. There was some fresh blood in the right nares. A mass could be seen posteriorly in the right side of the nose. Nasopharyngeal examination revealed a dark bluish-red blood-covered mass that filled the nasopharynx. There was also a right hemotympanum due to some blood in the right middle ear. Appropriate x-rays revealed a large mass in the nasopharynx with some clouding of the right maxillary and frontal sinuses.

Biopsy transnasally revealed fragments of necrotic tissue only.
