Twenty-two out of 37 patients with otosclerotic fixation of the stapes, demonstrated circumscribed swellings of the stria vascularis in the region of the active otosclerotic focus, adjacent to the spiral ligament. A vascular shunt was observed between the vessels of the otosclerotic focus and the spiral capillaries of the cochlea; this is regarded as the primary cause of the swellings in the stria. Five further cases of otosclerosis with fairly severe perceptive hearing losses and marked inner ear changes in the form of lamellar new bone formation and atrophy of the labyrinth also showed these circumscribed swellings of the stria in the region of the spiral ligament. In addition, the inferior spiral vein and the vein of the aquaeductus cochleae were both grossly dilated.
