The search for an autogenous graft to replace the stapes must continue.1-3 Elastic cartilage with covering perichondrium from the edge of the pinna, about the junction of middle and upper thirds, has been used in this series of 93 patients. The results are similar to methods using protheses.4,5
Cartilage from the pinna has an adherent perichondrium covered with fat. Fat also covers the free margin of foot of the graft. The cartilage has a suitable thickness for the oval window niche, and a useful convex curvature. A perichondrial flap made from the cut end anchors the cartilage graft to the incus and forms a capsule for the new joint between the lenticular process and the graft.
