THE PROBLEM of maintaining the patency of the nasofrontal duct after external operations on the frontoethmoid group of sinuses is probably the most annoying confronting the rhinologic surgeon. Since the advent of operations on the frontal sinus, numerous methods and procedures have been devised and suggested to solve this difficulty. No one method to date has met general acclaim. It was thought that lining the raw surfaces of the nasofrontal opening with skin or mucous membrane might be the proper approach. Mucosal flaps were suggested by Killian and Mithoefer. Killian¹ suggested cutting and dissecting downward to the floor of the nose a flap of mucosa from the superior turbinate before beginning the external incision. This flap is used to line the nasofrontal opening at the end of the operation. Mithoefer² advocated the Killian technique and suggested that in addition, if possible, a flap of mucous membrane be secured . . . [Full Text PDF of this Article]
