PLACE the patient in exactly the same position as for tonsillectomy under general anesthesia, with a sandbag between the shoulder blades and the neck fully extended. In this position the roof of the nose, from which the nerves come, is in the most dependent portion of the nose.
Soak a 2 inch (5 cm.) length of inch (6.5 mm.) ribbon gauze in a 5 per cent solution of cocaine hydrochloride containing epinephrine hydrochloride and squeeze out the solution well. Place this along the floor of the nose with dressing forceps. Lift it up well against the floor. There should be no difficulty in cases of polypi. If there is a bad deflection, a curved probe will help.
A pea-size blob of cotton wool dripping with the same solution is placed in the anterior naris. The nostril is gently squeezed at intervals of three to five minutes.
