IN THE discussion of the modified radical mastoidectomy I should like to say that I subscribe in a large part to the ideas expressed by Wittmaack in his discourse on pneumatization of the temporal bone.

Pneumatization of the temporal bone begins soon after birth, usually within the first twelve months, and is almost completed by the fourth or fifth year of age. Some pneumatization takes place all through life.

If infection of the middle ear takes place in infancy or before the end of the fourth or the fifth year of age, pneumatization is definitely disturbed, resulting in no pneumatization if the infection is early and of a severe type, or partial if the infection takes place after the normal pneumatization has begun or if the infection is of a less severe degree.
