The study presented by Brook et al¹ in the December issue of the ARCHIVES, appears similar in its approach to sinus disease. This approach was used before the advent of functional endoscopic sinus surgery and the modalities of treatment associated with it. Their study raises a number of questions, including what is chronic sinusitis and the authors' approach to treatment.

For more than 10 years we have used many of the principles introduced and established by David Kennedy, MD, not only concerning the surgical techniques of functional endoscopic sinus surgery but even more importantly his approach to the medical management of sinusitis. The duration and type of treatment employed in this study leaves one to question whether many of these patients actually had acute or subacute sinusitis and not true chronic sinusitis.
