This is in response to the letter by Stevens and Gray regarding the results of our survey on preoperative scanning and presumed thyroglossal duct cysts.

First, I want to emphasize that the primary purpose of our study was to survey and to present what the physicians in the community currently do and, only secondarily, sample their experience and exposure to patients with ectopic thyroid.

Certainly, there is great controversy about the need for preoperative scanning and/or other tests as evidenced by the results of our survey. Stevens and Gray are correct that the majority of physicians do not order nuclear scans. Cost containment is a major issue. I caution them that a cost analysis of this problem can be difficult.
