I read the article entitled "Cleft Lip and Palate Surgery in La Ceiba, Honduras" by Smith et al with enthusiasm.¹ These authors have underscored the enormous task of performing cleft lip and palate surgery in Third World countries, outlining the requirements for other professionals who wish to participate on overseas teams. They also emphasize that management of the problem necessitates the team approach—the accepted standard of care of these patients. The medical treatment provided by those of us who serve on these medical teams allows these children and adults to overcome the physical and functional problems of their condition and thus to lead as normal a life as possible. Because the treatment provides hope when perhaps none existed before, we often assume that we will be readily accepted and that people will trust us when we arrive in their countries with our equipment and skills in hand.
