A 77-year-old man presented with a 3-year history of an enlarging lump in the back of his neck. He denied pain or tenderness, previous trauma, rapid growth, or skin change. He also denied smoking, drinking alcohol, or exposure to radiation.
Physical examination revealed a 5x4-cm mass in the suboccipital region; the findings of the ear, nose, and throat examination were otherwise unremarkable. The mass was soft, cystic, and mobile, with no tenderness and no changes to the overlying skin.
At operation, an encapsulated soft mass was found in the suboccipital region, just deep to the subcutaneous tissues, and was easily dissected from the surrounding tissues. The histopathologic findings are illustrated in Figs 1 and 2.
