Dr Katz presents a thoughtful commentary on the current controversy over subspecialization in otolaryngology–head and neck surgery and certificates of added qualifications. He raises important questions that should be addressed in the context ofsociety's needs and calls for us to look at what our patients need, not what will give us a competitive market position.

We should take heed as we look at our specialty in the context of a reconfigured health care system. Generalism is in.
