A 69-year-old white man presented to the otolaryngology clinic with a 3-year history of slowly enlarging bilateral, cervical adenopathy. He denied weight loss, cough, hoarseness, and dysphagia. The findings of fine-needle cytology of the largest node were indicative of squamous cell carcinoma. Computed tomography demonstrated bilateral neck masses (Fig 1). The findings of panendoscopy and metastatic workup were negative, and the case was staged as a TX, N2, cM0 squamous cell carcinoma of the neck.
The patient subsequently underwent bilateral neck dissections. At the time of the left neck dissection, a smooth, ovoid mass measuring 0.50 x 0.3 x 0.2 cm was found within the carotid sheath. Figure 2 is a photograph of the gross specimen, and Fig 3 is a photomicrograph of the lesion.
