A 38-year-old black woman presented with a 4-year history of increasing nasal airway compromise. Systemic and topical nasal steroids were required to wean her from over-the-counter oxymetazoline hydrochloride nasal spray. A strict regimen of nasal steroid spray was only marginally successful in improving her nasal airway. Allergy tests were negative.
Examination revealed severe inferior turbinate hypertrophy, which responded poorly to vasoconstrictive spray. A fiberoptic examination revealed minimal turbi nate cobblestoning, a clear nasopharynx, and a normal larynx. A preoperative chest roentgenogram was unremarkable, and an uneventful bilateral inferior turbinoplasty with preservation of the medial inferior turbinate mucosa was performed.
