At the American Academy of Facial Plastic and Reconstructive Surgery meeting in Washington, DC, on September 23, 1988, Thomas C. McClure, MD, New York, NY, presented his technique in handling wide or underprojecting nasal tips using a modified Goldman approach. This differed from the standard Goldman approach in that Dr McClure advocated leaving the vestibular skin intact. He cautioned against resecting too much mesiocephalic margin of the lower lateral cartilages. The advantage of the Goldman tip is to increase the height of the nose and to achieve good narrowing as well. Dr McClure felt that the surgeon should avoid septoplasty via the open technique.
In summary, Dr McClure advocated minimal cartilage resection and leaving the vestibular skin intact, and that using this technique with individuals with thick skin yielded the maximum amount of narrowing.
