Improved Intraoperative Eighth Nerve Monitoring

A method to improve intraoperative eighth nerve monitoring was presented by Dr Paul Lambert, University of Virginia Medical Center, Charlottesville, at the Denver meeting of the American Neurotologic Society. The combination of electrocorticography and auditory brain-stem response (ABR) recording techniques was found to enhance recording of wave I. A new foam canal electrocochleography electrode provided a recording electrode as well as a sound delivery system for ABR. The electrode lowers electrical impedance improving the signal-to-noise ratio, which allows fewer averages and, thus, less time to resolve wave I. The enhancement in wave I was increased by approximately 65% in both normal hearing and hearing-impaired individuals. This is especially true in cases where wave I was difficult to distinguish by ABR. Although this monitoring technique optimizes information from various levels in the auditory system, it has not been documented that it will be able to significantly improve the degree of hearing
