In Reply.—Dr Gates raises several points in his letter that are not supported by the content of my commentary. For example, he seems upset at my use of the phrase "simple reassurance." This phrase, it should be noted, is a direct quote from Dr Goodhill's 1981 statements. Gates claims I asserted "cognitive therapy is something new" when in fact my article stated "Cognitive therapy has been used extensively for years." He states that my commentary implies "a large proportion of grossly dissatisfied patients with tinnitus." His experience indicates "a small (but persistently vocal) minority." Nowhere in my commentary are numbers of dissatisfied patients indicated. I agree that it is a small percentage who cannot cope after proper counseling. However, even a small percentage of patients with tinnitus represents a very significant number of individuals when one considers the total number of patients seeking relief.
