The lead article in our journal this month presents an astonishing report regarding the large number of very young children who begin to use smokeless tobacco on a regular basis. Although we are trained to read journal articles in a dispassionate manner, I must tell you that I cannot put aside entirely the mental juxtaposition of these tables and figures with the images of children playing and the head and neck cancer patients we see in our offices and operating suites.

As I read and reflected on its content, the need grew to share with you some thoughts and personal concerns about this growing health problem and our impotence as physicians to organize an effective campaign of public education and political action.
