In Reply.—It is important to note that our study did not represent our first sojourn in the field of antibiotic prophylaxis following head and neck surgery. We have previously reported a prospective randomized trial involving more than 100 patients.1 In both studies, cases were carefully stratified for variables that may have had an impact on the development of postoperative wound infection, such as stage of disease, prior tracheotomy, prior radiation therapy, and procedure. Subsequent to surgery, patients were followed up, and the status of their wound was graded according to an objective scale. It should be noted that no placebo group was employed in the preliminary study. At that time, we thought that prior studies, such as those of Becker and Parell2 did, in fact, offer support to a trial in which a placebo group was not used.
