In Reply.—Dr Whitesell raises an important issue, but it may not have the cause-effect relationship that he suggests. Much of the practice of primary care physicians involves "our" diseases, and a large percentage of the current practice of otolaryngologists is clearly primary care. I suspect that the majority of those of us who completed our specialty training within the last 20 years feel that we are using our skills below our highest capacity. I submit that it was ever so, and that it will continue to be so, as a generalization. Fellowships may be of higher quality or of lesser quality, but I doubt that they are a serious threat to the practicing otolaryngologist—head and neck surgeon.
