—I appreciate the opportunity to respond to Dr Lewinsky's letter. The intent of the publication was to describe a surgical reconstructive technique and not to discuss the management of plasmacytoma. The purpose of the article was explicitly stated not only in the abstract, but also in the introductory portions of the text. In an effort to make the article succinct, I elected not to include a description of the patient's rather lengthy medical history. My intentional omission apparently was a mistake, because it has led to some misunderstanding.
This man's original histopathologic diagnosis was that of undifferentiated malignancy which was treated with planned combined therapy, including a partial maxillectomy with postoperative radiation therapy of 5,000 rad. The patient remained free of any recurrent disease for 22 months at which time a local recurrence within the remaining maxillary sinus was noted, which resulted in a maxillectomy with orbital preservation.
