The article by Leopold and Lagoe1 represents a departure from the high-quality scientific reports we usually see in the ARCHIVES. This departure is welcome and overdue. The authors are to be commended not only for their extensive analysis on diagnosis-related groups (DRGs), but also for having the courage to submit their article to a peer-reviewed journal.

Otolaryngologists must become active participants in directing health care policy whether or not we desire to do so. I suspect that most of us prefer to do what we spent so many years learning to do—caring for patients. It is precisely this caring for patients that mandates our involvement! Do you believe the insurance carriers or our legislators have a nobler goal than cost reductions for health care?
