PATHOLOGIC QUIZ CASE 1

A 55-year-old woman was referred to me, after being followed up by an outside physician for one year, with persistent complaints of right-sided otalgia and right submandibular swelling. Several months after the onset of these symptoms, she noted pain at the base of the tongue. Previously, she had undergone an excisional biopsy of the right submandibular gland and an adjacent node, which disclosed only nonspecific inflammation. An otolaryngological examination at the time of referral showed a 1.5 x 2-cm firm submucosal mass located in the right side of the base of the tongue. The tongue was normally mobile. In addition, there was a 3.5 x 3.5-cm fixed mass in the right submandibular area. A chest roentgenogram was clear. A biopsy was performed. See Figs 1 through 3.
