A 46-year-old woman was aware of a progressive "loss of smell" during the previous three years. In the last three months, there was a sense of nasal fullness and a discharge that was occasionally blood tinged.
On physical examination, an ulcerated, beefy-red mass arose from the upper portion of the nasal septum and filled most of the nasal fossa. Destruction of the ethmoid sinuses and floor of the orbit was apparent on roentgenograms of the skull. An arteriogram showed a highly vascular tumor that bridged the cribriform plate and had a 2-cm intracranial component. A biopsy specimen is shown in Fig 1 and 2. An electron micrograph is shown in Fig 3.
