Dental Considerations in Median Labiomandibular Glossotomy

Recently, Wood et al1 advocated the removal of a healthy incisor tooth to facilitate the median labiomandibular glossotomy approach to the posterior region of the oropharynx. Under these circumstances, head and neck surgeons should exercise caution before extracting a tooth, since, in the interest of preserving healthy teeth, interdental osteotomies have been accomplished routinely and with good success by oral and maxillofacial surgeons for several years.2,3 When the median labiomandibular glossotomy approach is performed, the utilization of an oral and maxillofacial surgeon's expertise with interdental osteotomies can be beneficial. The use of a midline interdental osteotomy with this procedure prevents an unesthetic, edentulous space in the anterior mandible that is expensive and difficult to restore prosthetically.
