PATHOLOGIC QUIZ CASE 1

A 58-year-old man had a three-week history of right-sided nasal obstruction and recurrent epistaxis. There was no history of weight loss, smoking, allergies, anosmia, diplopia, or headaches. The patient is a mild diabetic with von Willebrand's disease. Pertinent physical findings were limited to the nasopharynx, where a large polypoid mass blocking the right choana was noted. The nasal cavities were free of any masses.

Paranasal sinus roentgenograms and tomograms (Fig 1 and 2) revealed an opacified right maxillary sinus with a large mass within the right turbinates. Chest roentgenogram and results of complete blood count, VDRL, and blood chemistry tests were within normal limits. The bleeding time was prolonged and the factor VIII level was low.
