PATHOLOGIC QUIZ CASE 1

David Larson, MD, Houston

A 39-year-old man had a history of recurrent sinusitis and nasal congestion for approximately ten years. The patient had a submucous resection in July 1973 and a revision of this operation in November 1973. He complained of anosmia since the first operation. He underwent an intranasal polypectomy in December 1974. One week prior to his present admission, the patient complained of left eye pain associated with left-sided rhinorrhea and fever. Culture showed Pseudomonas aeruginosa, and radiographic findings were compatible with left-sided pansinusitis. The patient denied any history of epistaxis.

Physical examination showed only a slight temperature elevation and tenderness of the left frontal and maxillary sinuses.

The patient began therapy with intravenously administered antibiotics and underwent a left-sided intranasal polypectomy and ethmoidectomy. A small portion of the tissue submitted is seen in Fig 1 and 2.
