A 45-year-old man presented with a 2-week history of an asymptomatic mass of the left parotid gland. He had no history of tobacco or alcohol abuse. A firm nodule was palpable immediately posterior and superficial to the angle of the left mandible on physical examination. A detailed neurologic examination revealed cranial nerves II through VII to be intact and symmetrical. Magnetic resonance imaging demonstrated a well-circumscribed nodule, measuring 2.6 cm in greatest dimension, in the superficial lobe of the left parotid gland (Figure 1, coronal gadolinium-enhanced T1-weighted image; Figure 2, coronal T2-weighted image), and the findings of fine-needle aspiration of the mass were consistent with a spindle cell neoplasm.
