A 20-year-old man presented with a 3-year history of an intermittently discharging sinus on the right side of his neck. The discharge was watery, and there was no history of pain, erythema, swelling, or fever. The findings of the physical examination were unremarkable, except for a small pigmented opening that was noted on the right side of the patients neck, just anterior to the sternomastoid muscle. Sinography was performed under fluoroscopic guidance, followed by computed tomography.
The sinogram demonstrated a linear contrast-filled tract (Figure 1 [arrow] and Figure 2 [single arrow]) extending from the external sinus in a posterior and superior direction to open in the pharynx at the level of the first cervical vertebra.
