A 71-year-old woman presented with obstructive sleep apnea syndrome. She complained of socially disruptive snoring and excessive daytime sleepiness. An overnight polysomnogram revealed severe obstructive sleep apnea with a respiratory disturbance index of 79 events per hour of sleep and a low oxygen saturation level (65%). A 1-year trial of continuous positive airway pressure was unsuccessful because of nasal irritation, recurrent epistaxis, discomfort, and claustrophobia despite multiple attempts to improve the fit and humidity of the mask. The patients medical history was significant for hypertension without known coronary artery or cerebrovascular disease. She refused to consider tracheostomy but was interested in uvulopalatopharyngoplasty.
Physical examination revealed moderate 2+ tonsils and an elongate soft palate that made contact at rest with the posterior pharyngeal wall. A pulsatile submucosal mass was noted in the midline of the posterior pharyngeal wall. . . . [Full Text of this Article]
