A 28-year-old otherwise healthy woman presented with subjective pulsatile tinnitus in her right ear. There was no history of otitis, surgery, or trauma. Otoscopy showed a curved, reddish formation behind an intact and mobile tympanic membrane in the right ear (Figure 1), with normal findings in the left ear. The results of all other investigations, including subjective and objective audiometric testing, were normal. Tympanometry revealed a pulsating movement of the tympanic membrane, with normal middle ear pressure. Computed tomography of the temporal bone (Figure 2) and magnetic resonance angiography with gadolinium (Figure 3) were performed.
