I agree with Dr Koch's hypothesis that the endoscopic browlift is preferable
to a traditional approach for most patients. I would further qualify this
statement as being true only for patients undergoing cosmetic surgery. Most
patients in my practice whom I consider candidates for endoscopic browlift
procedures are those seeking rejuvenative treatment of the upper third of
the face. I have not found the technique applicable to reconstructive problems
such as paralysis or severe asymmetries.

I find the endoscopic procedure an advancement and particularly valuable
in addressing the problem of mild to moderate brow ptosis in most patients
who seek upper blepharoplasty. When the brow is repositioned at a more desirable
level, the accompanying relative upper lid excess is reduced, allowing for
